Anatomy of a Real Mold Assessment: A St. Johns County Case File

They called us about a flooded closet. The flooded closet turned out to be the smaller problem.
Most mold assessments start with one specific thing the homeowner is worried about. A leak. A stain. A smell.
That one thing is almost never the whole job. It's the reason someone finally opens the door.
This post walks through a real assessment we recently completed at a single-family home in St. Johns County — what we were called for, what we actually found, how each finding was confirmed, and what went into the report. Every client-identifying detail has been removed. The measurements, lab results, and recommendations are unaltered. We've also published the anonymized assessment report and the anonymized remediation protocol in full, so you can see exactly what these documents contain before you ever pay for one.
The call
The air conditioning condensate drain backed up.
Not a slow drip — enough water to cause localized flooding in the air handler closet, which then spread into the adjoining bedroom closet. The homeowners got the drain corrected, dried what they could, and put a couple of store-bought moisture absorbers in the bedroom closet. Then they called us, roughly a week later, to find out whether the water had left them with a mold problem.
That's a good call to make, and it's the right timing. Wet building materials don't need long. In a Florida August, a week is plenty.
So the assignment, as it came in, was narrow: two closets, one known water event, one clear question.
Walking the whole house anyway
An assessment is visual and non-invasive — we don't cut walls open or pull flooring. But we walk everything accessible, not just the room you're worried about, and we document it in writing and in photographs. This inspection produced 33 photographs.
The two closets looked exactly like a water event should look.
In the primary bedroom closet: dark staining across the carpet field, heaviest along the wall lines, and a continuous band of green and dark fungal growth along the bottom edge of the baseboard where it meets the carpet — running along multiple walls and continuing through the inside corner. In the air handler closet next door: the baseboard on the left side had pulled away from the wall, with a visible gap and dark staining along its full length, plus growth on the leg of the air handler stand.

Left side wet, right side of the same closet dry. Water moved in one direction — toward the adjoining closet. Consistent with what the owners described.

Then we kept walking.
The finding nobody called about
In six other rooms — nowhere near either closet — there was dark speckling on the ceiling around the supply air registers, and on the register faceplates themselves.
The primary bathroom water closet. The linen closet. The kitchen. The living room. A guest bedroom. The guest bathroom faceplate had rust staining and debris built up behind the lowest louver. The dining room ceiling showed water damage right at the register.
This was not part of the complaint. The owners had seen it — it's visible if you look up — but it had never been the thing that made anyone pick up the phone. A drain backup is an emergency. Grubby-looking vents are something you mean to deal with eventually.
Here's why that distinction matters: the flooded closets were one water event with a known date and a fixed boundary. The registers were a pattern spread across six rooms, all fed by the same air handler. Two completely different problems, and the second one was the bigger one.
It only got looked at because something else got us in the door.

Moisture mapping
Before drawing any conclusions, we measure. And this is where a lot of homeowners get the wrong idea about moisture meters: a single reading tells you almost nothing. Readings mean something compared to each other.
Seven readings on the same carpet, in the same closet, on the same day:
99.9% — corner at the baseboard
99.9% — corner at the baseboard
92.6% — along the wall run
90.9% — corner area
33.4% — near the corner, adjacent to the growth
19.0% — interior of the closet
13.3% — interior of the closet
13.3% in the middle of the closet, 99.9% at the walls. That spread is the finding. It maps a wetted area with hard edges, and those edges line up precisely with where the growth was — along the walls, worst in the corners. That's water arriving from outside the room and wicking in, exactly as described.

Same approach in the air handler closet: left baseboard 56.4%, right baseboard not elevated at all. A localized wet area, not a general humidity problem.
And in a guest bedroom, where the occupants believed they had moisture in the carpet, the carpet was dry. Nothing. Ruling things out is as much of the job as finding things.
So the water event was real, bounded, and behaved the way the owners said it did. Which meant it could not explain the registers.
Taking the access panel off
This is the step that changes jobs, and it's the one most often skipped.
We removed the air handler access panel and looked inside the equipment. What was in there was heavy white and gray growth across the blower motor wiring — on the wire insulation, wire ties, connector housing, terminal block. More along an interior metal channel in the blower compartment, with staining beside it. More on the refrigerant line insulation and low-voltage wiring where they pass through the cabinet. Debris and accumulation across the evaporator coil fins.

That reframes everything. Growth in a closet is a closet problem. Growth inside the air handler is a whole-house problem, because that cabinet conditions and circulates air into every room in the building. Every one of those six ceiling registers is downstream of it.
This unit also had no return plenum — return air was being drawn from inside the AC closet itself. So the closet interior was functionally part of the air pathway. That's why the protocol later requires the entire closet to be cleaned to the same standard as the inside of the cabinet.
The homeowners called about water on the floor. The confirmed contamination was in the equipment above it.
Ruling out the usual suspects
We entered and inspected the attic. No roof leaks, no staining on the sheathing or framing, no water intrusion. Flex duct strapped and supported with no disconnections or tears. Both bathroom exhausts and the dryer vent terminating properly outside. Twelve inches of blown insulation.
A clean attic is genuinely useful information — it eliminates the most common alternative explanations for growth on a ceiling. Outside, the condensing unit had shrubbery grown in tight against the coil faces on multiple sides. An airflow restriction worth correcting, though not the cause.

Sampling
Four surface swab samples went to an accredited independent laboratory under chain of custody: the primary bedroom closet, inside the air handler, the baseboard in the AC closet, and the kitchen ceiling register.
Note what that sample set does. Two samples cover the water event, one covers the equipment, one covers the register pattern. Each of the three theories gets tested.
We chose surface swabs over air sampling deliberately. Visible growth was already present at identifiable locations, so the open question was "is the material on this surface actual growth?" A swab answers that. Air sampling answers a different question, about what happens to be airborne at the moment you sample.
All four came back GROWTH.
Identified fungal types across the samples: Alternaria, Aspergillus, Cladosporium, Curvularia, Penicillium/Aspergillus, chlamydospores, and hyphae.
Two things in that result deserve explaining, because homeowners almost never get told either one.
Hyphae were present in all four samples. Hyphae are the structural filaments of fungus — the root system, essentially. Finding a mass of hyphae on a surface means growth occurred on that surface. It's the difference between mold growing there and mold dust that drifted in and settled. All four locations were real growth, including the register.
No water-indicating fungi were found. Chaetomium and Stachybotrys — the genus most people mean when they say "black mold" — don't typically grow indoors unless building material has genuinely been soaked. Their absence in all four samples is a favorable finding, and we said so plainly in the report. It doesn't make the confirmed growth less real, but the homeowner earned that piece of good news.
And one point worth keeping if anyone ever tells you an air test "came back clean": where growth is confirmed on a surface by direct sampling, that evidence outweighs an air result. A non-elevated air sample would not have erased mold growing on the wiring inside that air handler.
Separating fact from opinion
A good report draws a hard line between what was observed and what is concluded. We label both.
Observed and confirmed: growth at four locations, lab-verified. Elevated moisture at specific measured points. Contamination inside the air handling equipment.
Professional opinion, stated as opinion: that the wet carpet and wet AC-closet baseboard came from the reported condensate backup — based on the reported timing, the position of the wet areas relative to the unit and to each other, and the absence of any other water source. And separately, that the growth at the supply registers is associated with how the HVAC system is running, most likely condensation forming where cold supply air meets warmer, humid air at the register boots and surrounding ceiling.
That second one is a hypothesis and it's written down as a hypothesis. The way you confirm or kill it is a full HVAC evaluation — including an actual Manual J load calculation, and an assessment of the system's humidity control and runtime, not just its cooling capacity.
Why that matters: an air conditioner oversized for the house will cool fast, satisfy the thermostat, and shut off before it has run long enough to pull moisture out of the air. The house gets cold and stays damp. That's a very common Northeast Florida story, and it cannot be diagnosed off a data plate. This unit's nominal capacity for 1,710 square feet looked unremarkable — but a square-footage ratio is not a load calculation, and we made no sizing determination in the report.
It's also, notably, the kind of condition that produces growth slowly and everywhere at once — which is exactly what six rooms of speckled registers look like.
The recommendations, in order
The report's recommendations were prioritized, and the order is deliberate:
HVAC evaluation first. Load calculation, humidity performance, airflow, and verification of the primary and secondary condensate drains, the drain pan, and the safety switch. Clear the shrubs off the condenser.
Correct the moisture. Confirm the backup is genuinely fixed and hasn't recurred. Dry or remove the wetted materials.
Remediate. Licensed remediation contractor, independent of us.
Verify. Independent post-remediation verification before containment comes down.
The condensate system leads the list for a reason. It already failed once — that's why we were called. Nothing else on the list is worth paying for until the thing that flooded the closet is confirmed fixed and the system that's feeding six rooms of registers is understood.
Remediation removes what grew. It does not remove why it grew.
The protocol is a separate document
The assessment report says what's there. It does not tell a remediation contractor how to do the work.
That's the remediation protocol — a separate deliverable with its own fee, and most licensed remediators will require one before they'll price or start a job. For this house it defined three Work Areas across seven pages: full six-mil containment with airlock entry for the air handler and the two closets, HEPA-filtered negative air exhausted outside, HEPA vacuums only, and removal criteria material by material — carpet, pad, and tack strip out entirely; duct board out, not cleaned; sheetrock cut back past the visible growth to the nearest framing member so the cut lands on sound material.
It also set the sequence, which is the part that saves real money: the air handler is remediated first. Clean the ceilings and registers while contamination is still sitting inside the equipment that feeds them, and the system will recontaminate the work as soon as it runs.
And it set the exit condition: containment stays up and the system stays off until an independent assessor performs post-remediation verification and issues a written passing result. Visual clean, moisture readings back in line with unaffected reference material, surface samples clear at the previously confirmed spots, and — because the air handler was a confirmed source — air samples run with the system operating, against an outdoor control.
Fail any of it and the contractor goes back in and it gets retested.

Why we don't do the remediation
Under Florida law (Chapter 468, Part XVI), mold assessors and mold remediators hold separate licenses, and the same company generally cannot do both on the same property. No referral fees, no financial relationship, no shared interest.
That isn't red tape. It's the reason a clearance result means anything. The person checking the work has nothing to gain from it passing.

What to take from this
The homeowners called about a flooded closet. They were right to. The water was real, the growth from it was real, and it needed to come out.
But the assessment they paid for to answer that question is what found contamination inside the equipment that feeds air to their whole house — and put a name on six rooms' worth of ceiling growth they'd been walking past.
If you're only ever going to look at the one thing that's obviously wrong, you'll only ever find the one thing that's obviously wrong.
Take the panel off. Or have someone take it off.
